How to report Notifiable Diseases in Georgia
Georgia physicians, laboratories, and other health care providers may report occurrences of certain diseases and conditions electronically, by mail or by fax. Timely and complete information, including laboratory results, speeds the investigation/intervention process. Treatment information is required when reporting sexually transmitted diseases.

- Electronically through the State Electronic Notifiable Disease

- Complete the Notifiable Disease/Condition Report Form available at http://health.state.ga.us/pdfs/epi/notifiable/reportingform.05.pdf
and mail, in an envelope marked CONFIDENTIAL, to:
