Phone: (770) 749-2511The Polk Primary Care Center serves low-income, uninsured Polk County residents. Costs are based on a sliding-fee scale according to income. Please bring proof of income for your appointment. Some payment is expected at time of office visit so we can continue providing these services to the Polk County community. The Polk Primary Care Center is made possible by a Rural Health Care Services Outreach Grant from the Health Resources and Services Administration and by local donations. Donations are tax deductible and very much appreciated.
Note: The Polk Primary Care Center is not for emergency care such as chest pain, bleeding, premature labor, delivery or trauma. For emergencies such as these, call 911 or go to the nearest hospital emergency room.
SERVICES
Non-emergency, bi-lingual, primary-care services offered without regard to race, color, national origin, age, sex or handicap include:
- Blood-pressure screening, treatment, management and follow-up
- Blood-sugar checks, diabetes management and nutrition education
- Treatmenat, management and follow-up for minor illnesses
- Cancer screenings and referrals
- Health-promotion and disease-prevention education
- Family planning and sexually transmitted disease (STD) screening
