STD Check: You will need to bring the following to your visit:- Identification (Drivers License, Baby Bracelets, Social Security Cards)
- Proof of household income Medicaid/PeachCare Cards
- Proof of address WIC voucher folder
- This service is available only to persons with no health insurance. Proof of Address, Identification (drivers license and/or Social Security Card) Proof of Household income. - TB Management ~ Active and Latent Infection

Immunizations: You will need to bring the following to your visit:- Identification

- These vaccines are provided through a special program for children with Medicaid, PeachCare, No Insurance or Under Insured.
- If the insurance does not cover vaccines, please bring your card for verification and documentation purposes. If your private insurance covers vaccines, they must be received at your doctor's office.

- Presumptive Medicaid (Pregnancy Medicaid). For Presumptive Medicate you must bring: Proof of Address, Proof of Household income, A copy your Birth Cerficate, Proof of Pregnancy which MUST be from a Dr's office/Medical Facility. Pregnancy Tests can be done on a walk-in basis. Identification (Divers License, SS card)
